READ CAREFULLY – THIS IS A LEGALLY BINDING DOCUMENT

In consideration for being allowed to participate in the 1v1 Freestyle Soccer Tournament (the “Event”), I, the undersigned participant (or the parent/guardian of a participant under 18), hereby acknowledge, understand, and agree to the following:

1. Assumption of Risk

I understand that participation in the Event involves physical exertion and inherent risks including, but not limited to:

• Sprains, strains, fractures, concussions, or other injuries

• Weather conditions, uneven surfaces, collisions, and accidents

• Potential exposure to illnesses, including but not limited to COVID-19

I voluntarily assume full responsibility for any risk of injury, illness, or other damages arising from my participation, whether caused by negligence or otherwise.

2. Release of Liability

I hereby release, waive, and discharge Bukky Visuals, the event organizers, sponsors, property owners, volunteers, and affiliates from any and all liability, claims, demands, losses, or damages related to personal injury, property damage, or wrongful death occurring before, during, or after the Event.

3. Medical Treatment

I certify that I am physically able to participate in this activity. If medical treatment is needed, I authorize any licensed physician or emergency medical personnel to provide necessary care. I understand that I am solely responsible for any costs incurred.

4. Photo/Video Release

I grant permission for Bukky Visuals or event organizers to use photographs or videos taken during the Event for promotional, advertising, and commercial purposes, without compensation or approval.

5. Minor Participants

If the participant is under 18 years of age, I, as the parent or legal guardian, give my consent for their participation. I agree to the above terms on their behalf and assume full responsibility for any resulting liability.

6. Rules & Conduct

I agree to abide by all event rules, directions from staff, and to demonstrate good sportsmanship. I understand that failure to follow rules or engage in unsafe behavior may result in disqualification without refund.

7. Legal Acknowledgment

This agreement shall be governed by the laws of the State of California. If any portion is held invalid, the remainder will remain in full effect. I have read and fully understand this waiver and voluntarily sign it.
